Not if it is done correctly. Every change is built as a separate Frappe app with its own version control, and core files are never edited. Before a version upgrade the custom app is tested against the new release on staging, and only then is the live system moved.

An example

The insurer's format prints without editing, and the stock issued to the patient has already left the ward sub-store. In the afternoon the medical director opens a revenue-by-consultant report that standard ERPNext never had, and the finance head opens the same figures by department. Nobody has exported anything.
